Lifecycle of a write

The leader's write is checkpointed and adopted by the follower. Its covered ingest table copy can be removed when no longer needed by readers.

Each loop repeats an independent write. Checkpoint adoption makes the ingest table copy eligible for eviction; removal waits until it is safe.
